By registering for any Program or activity with Triva Emery, BeWell-MV sponsored by Cape Cod Municipal Health Group, I hereby assume all risk of injury or harm as a result of the activities specified above and agree to release, indemnify, defend, and forever discharge Triva Emery, BeWell-MV from all liability, claims, demands, damages, costs, expenses, and causes of action due to death, injury, loss, or damage. I understand all BeWell-MV Programming and Cape Cod Municipal Health Group activities are offered at no cost to me and I voluntarily participate.
